Clozapine Black Box Warnings: Enhancing Safety and Knowledge in Antipsychotic Therapy

Abstract:
This study aimed to evaluate pharmacy students' understanding of clozapine, an atypical antipsychotic drug used to treat schizophrenia, particularly in cases resistant to other treatments. Given the serious risks associated with clozapine, such as agranulocytosis, the study sought to assess students' knowledge of its black box warning, safety protocols, and ability to manage patient safety effectively.
A survey was conducted among 46 first-year pharmacy students at Howard University College of Pharmacy, with 43 participants (93.5% response rate) completing the questionnaire. The survey was designed to capture both opinion-based and knowledge-based data, focusing on the students’ awareness about risks of clozapine therapy and their familiarity with related safety protocols. The results showed a significant awareness of the drug's severe risks and yet revealed notable gaps in the students' in-depth understanding of the specific safety measures required for its use. The study revealed that while most participants had a general awareness of clozapine's associated risks, they lacked detailed knowledge regarding its monitoring requirements. Limitations included the small sample size and the absence of long-term clinical exposure to clozapine for most students. These findings underscore the need for enhancing pharmacy curricula with more targeted education on high-risk medications, ensuring that future pharmacists are better equipped to manage complex cases of schizophrenia and other psychiatric disorders.
